column_border_style=&#8221;solid&#8221; bg_image_animation=&#8221;none&#8221;][divider line_type=&#8221;No Line&#8221;][vc_column_text]\n<h2>Short-term Disability<\/h2>\n<p>National Short-term disability program began on January 1, 2017 with program updates effective\u00a0October\u00a01, 2018.\u00a0<span data-ccp-props=\"{&quot;134233117&quot;:true,&quot;134233118&quot;:true,&quot;201341983&quot;:0,&quot;335559740&quot;:240}\">\u00a0<\/span><span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:0,&quot;335559737&quot;:-135,&quot;335559739&quot;:200,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p>As the ELCIC benefits plan sponsor, GSI has arranged a disability plan that reflects the caring and compassion of ELCIC employers demonstrated by:<\/p>\n<ul>\n<li>Providing an income replacement plan for eligible disabled employees,<\/li>\n<li>minimizing the stress associated with the disability application process for both the employer and employee,<\/li>\n<li>getting an employee back to work as quickly as possible where medically appropriate in an effort to live a full, healthy and productive life.<\/li>\n<\/ul>\n[\/vc_column_text][toggles style=&#8221;default&#8221;][toggle color=&#8221;Default&#8221; title=&#8221;Purpose&#8221;][vc_column_text]This\u00a0program\u00a0provides income protection to ELCIC Plan Members in the event they are unable to work due to illness or injury and to support a safe, sustainable and early return-to-work that is effectively and medically supported.\u00a0The policy aims to create an environment of wellness and engagement through awareness and acceptance of any health challenges, to\u00a0support\u00a0health and wellness promotion and to have excellent attendance and engagement.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>The policy outlines the circumstances and criteria wherein Plan Members may be eligible for STD benefits while ensuring that all\u00a0Plan Members, regardless of their length of service, are treated equally and equitably.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>Information regarding how to apply for and the compensation and benefits entitlements during STD claims\u00a0is provided in this document.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>This is an employer-funded benefit. STD claims and benefit eligibility are reviewed and assessed with the support of an\u00a0independent\u00a0disability management firm.\u00a0ELCIC Group Services Inc. (\u201cGSI\u201d)\u00a0maintains approval authority for compensation and\/or pay treatment.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>Each case is unique and will be considered on a case-by-case basis, taking into account the circumstances and complexities of the situation.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Definitions&#8221;][vc_column_text]<b><i>Actively at Work<\/i><\/b>\u00a0\u2013 Means a Plan Member is doing work tasks as assigned by their Employer, including paid time off or weekends (or two pre-designated equivalent days).\u00a0 Plan members away from work on an Approved Leave of Absence are considered to be Actively at Work within the limits noted under the Continuation of Coverage section. Plan members who are away from work for leave other than Approved Leave of Absence, paid time off, or weekends are not considered to be Actively at Work.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p><b><i>Approved Leave of Absence<\/i><\/b><b>\u00a0<\/b>\u2013<b>\u00a0<\/b>The following leaves, provided they were applied for and approved as appropriate by\u00a0the Plan Member\u2019s\u00a0Employer\u00a0are considered Approved Leaves of Absence\u00a0for the purposes of this Plan: vacation,\u00a0personal\u00a0leave of\u00a0absence including sabbaticals,\u00a0and statutory leaves of absence\u00a0such as maternity or parental\u00a0leave, bereavement,\u00a0jury\u00a0duty,\u00a0or compassionate\u00a0care\u00a0leave.\u00a0 Other situations may be considered with GSI\u2019s prior written approval.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p><b><i>Totally Disabled<\/i><\/b><b>\u00a0<\/b>\u2013 A Plan Member is considered Totally Disabled when, immediately following a period of being Actively at Work,\u00a0they are unable to perform the usual required duties of their Regular Occupation or any reasonable accommodated or modified duties provided by the Employer.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p><b><i>Plan Member<\/i><\/b>\u00a0\u2013 An eligible\u00a0employee who is enrolled as a Plan\u00a0Member in the ELCIC Group Benefits Plan.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p><b><i>Employer<\/i><\/b><i>\u00a0<\/i>\u2013 An organization that has signed a subscription agreement for the ELCIC Group Benefits Plan administered by GSI.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p><b><i>Regular Occupation<\/i><\/b>\u00a0\u2013 The Plan Member\u2019s normal and habitual role, as described in their job\/position description and\/or employment agreement.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p><b><i>Salary Basis<\/i><\/b>\u00a0\u2013 The amount of salary and housing\u00a0allowance\u00a0which the Plan Member was receiving immediately prior to the start of the Total Disability, including\u00a0additional regular employment\u00a0earnings\u00a0such as\u00a0bonus\u00a0and\u00a0overtime\u00a0as reported on the Salary Basis Calculation form.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Benefit Amount&#8221;][vc_column_text]<span class=\"TextRun SCXW92792582\" lang=\"EN-US\" xml:lang=\"EN-US\"><span class=\"NormalTextRun SCXW92792582\">The benefit amount is 70% of Salary Basis (effective on the last day Actively at Work<\/span><\/span><span class=\"TextRun SCXW92792582\" lang=\"EN-US\" xml:lang=\"EN-US\"><span class=\"NormalTextRun SCXW92792582\">)<\/span><\/span><span class=\"TextRun SCXW92792582\" lang=\"EN-US\" xml:lang=\"EN-US\"><span class=\"NormalTextRun SCXW92792582\">. Required contributions from the ELCIC Pension Plan will be deducted from and based on the Benefit Amount.\u00a0 The Employer will also make the required pension contribution based on the Benefit Amount.\u00a0<\/span><\/span><span class=\"EOP SCXW92792582\" data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Waiting Period&#8221;][vc_column_text]Members\u00a0will be paid during the waiting period for STD benefits, in accordance with the <a href=\"https:\/\/dev.elcicgsi.ca\/?page_id=1912\" target=\"_blank\" rel=\"noopener noreferrer\">National Compensation Guidelines\u00a0Sick Leave Policy<\/a>\u00a0or\u00a0their\u00a0Employer\u2019s corporate policy.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>STD\u00a0benefits are payable\u00a0after\u00a014 consecutive days of absence due to medically supported illness or injury.\u00a0STD\u00a0Benefits will be\u00a0payable\u00a0if the STD claim is deemed eligible and\u00a0medically\u00a0supported.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Duration of Short-Term Disability Benefits&#8221;][vc_column_text]The maximum duration of the STD period is\u00a017\u00a0weeks from the first day of medically supported absence of illness or injury:\u00a0<span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<ul style=\"font-weight: 400;\">\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"25\" aria-setsize=\"-1\" data-aria-posinset=\"1\" data-aria-level=\"1\">2 weeks\u00a0waiting period\u00a0covered by Sick Leave Policy, followed by<span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/li>\n<li data-leveltext=\"\uf0b7\" data-font=\"Symbol\" data-listid=\"25\" aria-setsize=\"-1\" data-aria-posinset=\"2\" data-aria-level=\"1\">15 weeks maximum covered by this Policy<span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/li>\n<\/ul>\n<p>Beyond\u00a0this\u00a017-week period, a Plan Member may be eligible for the Long-Term Disability benefits\u00a0under the ELCIC Group Benefits Plan.<span data-ccp-props=\"{&quot;134233279&quot;:true,&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Coverage for Short-Term Disability Benefits during leaves of absence&#8221;][vc_column_text]<span class=\"TextRun SCXW214105359\" lang=\"EN-US\" xml:lang=\"EN-US\"><span class=\"NormalTextRun SCXW214105359\">Plan Members who are on Approved Leave of Absence will continue to be covered by the STD Plan and their Employer will be required to continue premium remittance. During a continuation of coverage, all other Plan provisions apply, and benefits are only payable after the date of the Plan Member\u2019s scheduled return from Approved Leave of Absence.\u00a0<\/span><\/span><span class=\"EOP SCXW214105359\" data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Gradual Return-to-Work&#8221;][vc_column_text]Every\u00a0possible effort will be made to provide suitable return to work (RTW) options for\u00a0Plan Members\u00a0who are unable to perform their regular duties, in keeping with the commitment to providing a safe workplace.\u00a0Consideration will be given to the\u00a0Plan Member&#8217;s ability and skills, in order to identify their\u00a0capabilities and\u00a0any requirements for accommodation.\u00a0The intent is to provide\u00a0a\u00a0temporary\u00a0solution until the Plan Member is\u00a0able to return to regular\u00a0duties.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p>During a period of modified work,\u00a0regular salary pro-rated for the time worked will be paid, plus\u00a0pay at the\u00a0STD\u00a0rate for the remaining time.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p>Example 1:<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p>An employee whose normal work schedule is 8 hours per day is only able to return to work for 4 hours per day to start. The employee would receive 4 hours of regular wages + 4 hours of STD benefit.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p>Example 2:<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:240}\">\u00a0<\/span><\/p>\n<p>An employee whose normal work schedule is 8 hours per day is able to return to full hours, but has a restriction of not lifting more than 10 lbs.\u202f The employee would receive their full regular pay, but their STD file will remain open until they are able to return to their regular job duties.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:240}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;Recurrence&#8221;][vc_column_text]In the event a Plan Member is absent from work because of a recurrence of the same or closely related disabling condition within a period of 30 calendar days following return to full pay, the recurrence will be considered a continuation of the previous disability.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>If the Plan Member\u00a0is absent because of the same disabling condition after 30 calendar days following return to full pay,\u00a0the absence will be considered a new STD claim with a new date of absence and medical support.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>If\u00a0Total\u00a0Disability occurs due to a new cause any time following the Plan Member\u2019s return to full pay, it will be considered a new STD claim.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span>[\/vc_column_text][\/toggle][toggle color=&#8221;Default&#8221; title=&#8221;STD Benefits Application Procedures and Administration&#8221;][vc_column_text]A Plan Member requesting benefits under the STD Plan must follow the procedures and requirements outlined in the STD Guideline to apply for and receive benefits.\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>The Plan Member is responsible for the payment of any costs associated with the provision of medical reports or completion of medical forms that may be required to initiate the STD claim application.<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>Plan Members\u00a0must ensure personal contact information remains up to date and current, including temporary changes, while on STD leave.\u00a0\u00a0<span data-ccp-props=\"{&quot;201341983&quot;:2,&quot;335559739&quot;:240,&quot;335559740&quot;:260}\">\u00a0<\/span><\/p>\n<p>The STD Plan is administered\u00a0by GSI.
